The partners in the project were the ICB’s Personalised Care Team, the My Life Choices lived experience panel, the Patients Association and PIF. 

Over the course of six co-production meetings we developed simple resources to support patients and professionals to have better shared decision making conversations. This webinar shares the findings of the project.
